What is Corinker and what is it used for?
Corinker contains nifedipine, a medicine that belongs to a group of medicines called calcium antagonists and is used to treat certain types of coronary heart disease and high blood pressure.

Corinker is used to treat:

high blood pressure (essential hypertension)
chronic stable angina (chest pain).
With high blood pressure: Corinker works by relaxing and dilating blood vessels. This allows the blood to move more easily and lowers blood pressure. Lowering blood pressure reduces the strain on your heart.

In angina (angina pectoris): Corinker works by relaxing and dilating the blood vessels in the heart. This allows more blood and oxygen to reach the heart and reduces its load. Angina attacks are milder and less common if the workload of the heart is less.

2. What you need to know before you take Corinker?

Do not take Corinker:

if you are allergic to nifedipine, other similar medicines (dihydropyridines) or to any of the other ingredients of this medicine (listed in section 6) during pregnancy;
when breastfeeding. If you need to take Corinker, you must stop breast-feeding before you start taking this medicine;
if you have ever had a collapse (expressed by shortness of breath, paleness, cold sweat and dry mouth) caused by heart problems (cardiogenic shock); in high-grade aortic stenosis (narrowing of the aortic valve); in unstable angina pectoris (chest pain);
if your blood pressure continues to rise despite treatment (malignant hypertension);
in an acute heart attack known as a heart attack (within the first 4 weeks);
in combination with rifampicin (antibiotic);
if you have an acute angina attack. Corinker does not respond quickly to the symptoms of angina.
if you have liver disease that prevents your liver from working properly.
if you have inflammation of the gut or Crohn's disease.
if you have a blockage or narrowing in your bowel, or have had this in the past.
Warnings and precautions
Talk to your doctor, pharmacist or nurse before taking Corinker.

The following describes in which cases you can take Corinker only under special circumstances and with special care. Please consult your doctor about these issues. This also applies if what has been written has applied to you in the past.

Particular care should be taken if:

you have low blood pressure below 90 mmHg and Corinker has been prescribed to treat angina;
you have heart failure that cannot be adequately treated (decompensated heart failure);
you are diabetic; The treatment for your diabetes may be adjusted;
you are on dialysis and have high blood pressure and decreased circulatory volume (hypovolaemia), Corinker may cause a sudden drop in blood pressure;
Your liver is not functioning properly; Your doctor may do blood tests. You may be prescribed a lower dose of Corinker.
Tell your doctor before taking the next dose of Corinker if:

your chest pain worsens or occurs more often or more severely after a period of several hours or days. Your doctor may advise you not to take Corinker.
you have chest pain after taking the first dose of Corinker. Your doctor may decide to change your treatment;
notice that your shortness of breath deepens
notice swelling of the ankles.
Also tell your doctor:

If you are about to have a urine test. Nifedipine may affect the results of some urine tests.
If you are about to have a barium X-ray test (barium porridge). These tablets may affect the results of the test.
Children and adolescents
Corinker is not recommended for use in children and adolescents below 18 years of age, as limited safety and efficacy data are available in this age group.

Other drugs and Corinker
Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines.
The effect of treatment may be affected by the following active substances or groups of medicines taken at the same time as Corinker. Tell your doctor if you are taking:

other medicines to lower high blood pressure.
rifampicin (a medicine used to treat tuberculosis).
cimetidine (used to treat stomach and duodenal ulcers).
digoxin, diltiazem, quinidine or beta-blockers (used to treat heart disease).
quinopristine / dalfopristin (combined antibiotic).
phenytoin, carbamazepine or valproic acid (used to treat epilepsy).
cisapride (used to treat disorders of the esophagus and stomach).
erythromycin (antibiotic).
magnesium sulphate given by injection during pregnancy (may cause a drop in blood pressure).
ketoconazole, itraconazole or fluconazole (antifungal drugs).
indinavir, nelf

Corinker with food, drinks and alcohol
You can take Corinker with or without food
Do not start taking Corinker for at least 3 days after drinking grapefruit juice or eating grapefruit. Tell your doctor if you have eaten grapefruit or drank grapefruit juice during this period. Do not drink grapefruit juice or eat grapefruit while taking Corinker. Grapefruit juice can increase blood levels of nifedipine. This effect can last for at least 3 days after eating grapefruit.

Pregnancy, lactation and fertility
If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.

Pregnancy
Corinker should not be used during pregnancy unless the woman's clinical condition requires treatment with nifedipine. Nifedipine should be used in women with severe hypertension who do not respond to standard treatment.

Breastfeeding

You should not take Corinker while breast-feeding. If your doctor decides that treatment is absolutely necessary, then breast-feeding should be stopped before you start taking Corinker.

Fertility
If you are a man who has not been able to become a father by in vitro fertilization, keep in mind that drugs such as Corinker can damage sperm function.

Driving and using machines
Corinker treatment can make you feel dizzy, weak, very tired or cause visual disturbances. Do not drive or use machines if you are affected in this way. This is especially true at the beginning of treatment, when changing the drug, as well as with the simultaneous use of alcohol.

3. How to take Corinker
Always take Corinker exactly as your doctor has told you. If you are not sure, talk to your doctor.
Corinker treatment can last indefinitely. Its duration is determined by the attending physician.
The dosage is determined by the attending physician strictly individually according to the severity of the disease and the response to treatment. Depending on the relevant picture of the disease, with constant monitoring of your condition, the appropriate dose should be determined for you.

High blood pressure
The starting dose is 1 tablet (20 mg) once a day. Your doctor may gradually increase the dose to a maximum of 4 tablets (80 mg) a day.

Stable angina pectoris:
The starting dose is 1 tablet (20 mg) once a day. Your doctor may gradually increase the dose to a maximum of 4 tablets (80 mg) a day.
Patients with limited hepatic function should be closely monitored and the dose may need to be reduced.
Adult patients may be treated with lower doses.

Use in children and adolescents
The use of Corinker in children and adolescents under 18 years of age is not recommended due to insufficient data on safety and efficacy in this age group.

When and how should you take Corinker?
The tablets should be swallowed whole (under no circumstances should they be chewed or broken), with a glass of water, with or without food. Intake during meals leads to delayed but not reduced absorption of the drug.
Do not take Corinker with grapefruit juice.

The recommended interval between 2 intakes is about 12 hours (morning and evening), preferably at the same time.

Elderly patients, patients with liver and kidney disease
The absorption of nifedipine may be altered in elderly patients so that a satisfactory effect is achieved with lower doses.
Nifedipine is metabolised mainly by the liver, so the dose should be determined by the physician based on the patient's liver function.
No dose adjustment of nifedipine is required in renal impairment.

If you take more Corinker than you should:
If you take large amounts of Corinker, you may experience a dose-dependent drop in blood pressure, a fast or slow heart rate, high blood sugar (hyperglycaemia) or high blood acidity, accumulation of fluid in the lungs (pulmonary edema). , reduced blood supply to vital organs and blurring of consciousness to coma.

If an overdose is suspected, contact your doctor immediately or go to the nearest hospital. For the treatment of overdose, medical professionals will be guided by the symptoms (such as vomiting may be induced, gastric lavage may be performed, and the stable condition of the cardiovascular system may be restored).

If you forget to take Corinker
Take the missed dose of Corinker immediately, still adhering to your dosing schedule, observing a 12-hour interval before taking the next dose. If in doubt, seek medical advice.
Do not take a double dose to make up for a forgotten tablet.
